Well, of the drug of which I will write today is considered a drug of abuse, we would first have to know that it is a drug of abuse, colloquially called a drug that refers to a non-medical substance with psychoactive effects. able to create changes in perception, mood, conscience and the way of behaving each individual can be self-administered.

Krokodil or desomorphine is a homemade drug based on opiates that is up to 10 times stronger than heroin and much cheaper and toxic. The effects of this drug are terrible. After injected, the skin of the area where the drug enters the body begins to turn gray and scaly, the most serious consequences come later, when after a few months of use, the skin and muscle tissue begin to rot, then detach and leave the bones exposed. When this consequence begins the skin of the person who consumes it becomes scaly and greenish and from there comes the name krokodil I already know that it could assimilate the skin of the individual to that of a crocodile

Administration routes

The main routes of consumption of krokodil are oral and parenteral (subcutaneous, IV or in the femoral artery mainly).

Effects

Desomorphine produces CNS depression. It is more addictive than heroin, its effect is shorter but 10 times more powerful.
Krokodil users experience euphoria only for about an hour and a half, while the pharmacological effects of heroin use can last from 4 to 8 h. The action of short duration and the time of less than an hour required for the home preparation of Krokodil makes its addicts, who prepare the drug themselves, get caught in a daily cycle of 24 hours of cooking and injection in order to avoid the withdrawal syndrome.
Parenteral overdose produces the classic triad of miosis, decreased level of consciousness (including coma) and respiratory depression.

Complications

Because this mixture is injected routinely practically without any purification process, the "krokodil" has gained a terrible popularity to be responsible for the production of very serious tissue damage: phlebitis and gangrene, thrombophlebitis, abscesses, infections in the bones, sores and ulcers on the forehead and skull, as well as necrosis in ears, nose and lips and liver and kidney problems.

The harmful effects of krokodil are mainly related to desomorphine, but also to the toxic components used to make it, such as iodine, which produces lesions of the thyroid gland and cartilage. Neurological and other endocrine organs have also been described.

The soft tissue damage occurs mainly around the injected areas and is caused by the accumulation of the drug. The mixture also seems to accumulate in the veins, since it can not completely dissolve in the blood. These accumulations necrotize the tissues and make their way to distant places in the body, widening the damage caused.
Since the effect of krokodil is quite ephemeral, this induces its users to inject several times per day, increasing even more the damage caused. The degree of damage to tissues is so high that the life expectancy of addicts to this mixture is usually as low as two or three years.

Mechanism of action

Its effects can be thrombosis and bleeding, large ulcers on the skin, phlebitis and gangrene in the area where the drug was injected, skin infections that reach the bones, loss of limbs, such as fingers, legs, arms, detachment of the skin in areas of dead tissue, ulceration of the veins, pneumonia, septicemia, bursting of the arteries, meningitis, loss of teeth, bursting of ears, nose and lips, renal and hepatic failure, speech problems, problems with motor coordination, failures in concentration and memory, personality changes.

In addition to the multiple physical injuries it causes, the drug is highly addictive. Its effect is stronger than that of heroin but lasts less time approximately 40 minutes, which causes addicts to resort to more doses. Because of its low cost and because it is not an illegally produced drug, its components are easy to reach, it makes more followers.

In the psychic part we have that the krokodil is an opiate and the opioids act by adhering to specific proteins called opioid receptors, which are found in the brain, spinal cord, gastrointestinal tract and other organs in the body. When these drugs adhere to their receptors, they reduce the perception of pain. Opioids can also cause drowsiness, mental confusion, nausea, constipation and, depending on the amount of the drug consumed, can depress breathing

Treatment

From acute poisoning: Ensure ABC. Symptomatic treatment of complications. Antidote: naloxone.
The treatment of complications of krokodil consumption requires, in most cases, surgical interventions of extreme complexity, such as the removal of major veins in the arms or legs, sometimes requiring amputation or skin grafts.

It could be thought that as the drug gives the same effects as heroin could cause the same consequences but through the investigation the consequences would be much more serious either by losing a limb or life expectancy low, all consumption of drugs is harmful to health and as people we must avoid at all costs that a dependence on a substance affects us until we reach death by it.
