Nice work! I got as far as hunting for the patient provider image needed for the overlay key, then "work" got in the way LOL. I started over in images before standard web results though and kinda went astray.

I still haven't worked out the second hint but I am thinking it probably ties into the first clue. Scouring the website to see if it offers any hints.

I was hunting through the related images from google search - things that look like the way the game is aligned. To put the number image over it like a key , to get the password. But no luck yet.

Well @timsaid, my brain is sore, didn't know that was possible. :-) Another incredible challenge, I'm happy I got a piece of it figured out this time. I will check back a bit later to see how everyone is progressing. Thank you for taking the time to create these for us.
The numbers almost seem to line up in a grid. (I added an extra row at the bottom because the empty space must have been there for some reason.)
Summing each column makes, in order,

The rows add up to
16 11 16 12 0
It's notable that both lists each have just one odd number.
