Revit MEP - Cleanout Annotation Not Appearing In One View

I guess to get more views maybe I should include some pics but hopefully it helps one person then it's worth it :-)

For the clean outs on my drainage they look like a clean out in 3D but on plan they show as just two lines which is the typical symbol we use. On my level two plan no lines? But the cleanout was showing in 3D. Hmmmmm.

View templates controlled many things in the view so my possibilities were narrowed since the level other levels were fine. I tried view range, hidden elements, then I saw the underlay was set to level 1. It worked! Why underlay would hide these lines I am not 100% sure. Any ideas?
