Maternal Mortality Rate (MMR) has become an important indicator in determining public health status. One of the main priorities in health sector development as contained in Propenas and strategies of Making Pregnancy Safer (MPS) or the pregnancy is safe as a continuation of the Safe Motherhood Program with the objective of accelerating the reduction of maternal morbidity and mortality and neonatal (MDG 2010), in a statement published on the official website of WHO explained that to achieve the Millennium Development Goal's target, the decline in maternal mortality from 1990 to 2015 should reach 5.5 percent per year.

One of the causes of maternal morbidity and mortality is postpartum infection where the infection starts from the perineal rupture. Perineal rupture can occur due to spontaneous rupture and perineal episiotomy performed on indications include: large baby, rigid perineum, labor abnormalities, delivery by using both forceps and vacuum devices. Because if the episiotomy is not performed on indication under unnecessary circumstances with the above indications, it causes an increased incidence and severity of damage to the heavier perineal region.
