Cervical radiculopathy is the radiating pain in one or both upper extremities due to compression or irritation in one or more cervical nerve roots. The pain may be acute or chronic depending on the patient's duration of pain initiation. The commonly involved nerve root values are C6 and C7. Mainly, incidence occurs in 50 or more years.

We have 7 vertebrae in the cervical region, and it's the sensitive region where compression can cause severe damage. But we have 8 cervical nerve roots. Any damage to the cervical nerve root or vertebrate can lead to cervical radiculopathy, or any other disease. C6, C7 perform wrist extension and flexion respectively.

Symptoms

The common symptoms related to cervical radiculopathy are radiating pain, numbness, and tingling sensation that travels from neck to arm and hand region. Sometimes, patients feel severe pain and numbness in their hands and fingers, and they are unable to perform any work.

Causes

Cervical radiculopathy can be caused by

  • Disc herniation
  • Compression fracture
  • Osteoarthritic changes
  • Spondylosis
  • Biochemically induced radiculopathy
  • Trauma
  • Tumor

Assessment

For the assessment of cervical radiculopathy, we perform the following investigations to make the diagnosis.

  • NCS
  • MRI
  • CT scan
  • EMG

In physiotherapy, we perform some special tests to confirm the diagnosis. The first one is the spurling test which provoke pain, the shoulder abduction test that relieve pain; and the cervical spine tests.

Physical Therapy Intervention for Cervical Radiculopathy

Physical therapy intervention for cervical radiculopathy are from weeks to months to regain normal ranges and subside pain with movement or at rest. Here are the few exercises videos that you have to perform to treat cervical radiculopathy patients. Treatment protocol vary from patient to patient. But the main thing is you have to evaluate the exact cause of cervical radiculopathy either it's age related or trauma.

Isometric Neck Flexion

This the exercise to strengthen muscles around your neck especially muscles that help in neck flexion, also improve your neck flexion.

Isometric Cervical Rotation

This exercise is perform to improve neck rotation as patient feel difficult to complete the range of motion due to pain and restricted ROM. Perform this exercise on both sides right & left to improve rotation on both sides.

Isometric Neck Extension

Isometric Neck Extension are perform to improve neck extension as sometimes it's difficult to extend your neck due to pain your ROM is restricted. Also muscles need strengthening.

Isometrics Neck Retraction

Isometric Neck Retraction is the exercise in which patient use arms muscles along with neck muscles. It's also a stretch of pectoralis major muscle as we place our arms behind the head.

Trapezoius, Scalene, Levator Scapulae Stretch

These are the best exercise to relieve pain and improve your shortened muscles as muscles reduced their length in pain phase to compensate. You can perform any of 2 stretches for your weekly lesson.

5 to 10 minutes heat up before exercise is necessary as muscles relax after heating then you can perform exercise easily. Also do soft tissue mobilization before performing these exercises to reduce pain. Perform 5, 5 repetitions on first week then increase accordingly. These are the exercises for beginners as other exercises can by performed by professionals and experienced physiotherapist or gym trainer.
