Hello, Friends!

Welcome to my blog. I will be making my entry on the weeks topic on rotator cuff tendonitis.

What's rotator cuff tendonitis? Write in your own words after getting knowledge from the lesson post. (Note: don't add special tests or investigations and detailed treatment in this question. Write the answer precisely).

The rotator cuff is a combination of four (4) muscles that are found within the shoulder region. And this comes with some surrounding tendons that collectively aid movement and stability within the entire area. The supraspinatus, Infraspinatus, Teres-Minor, and Subscapularis muscles are known to guarantee different roles, but all are tailored towards movement and stability of the region.

Therefore, Rotator Cuff Tendonitis is a condition that affects the rotator cuff where we observe inflammation or relative damage on those surrounding tendons that support the activities of the major four muscles within the shoulder region. This inflammation may be caused by varying factors, which are all considered as primary stimulators to the rotator cuff that causes Tendonitis.

Causes:

Rotator Cuff Tendonitis, as I said, is caused by varying factors that include inflammation from repetitive use of the arm, especially overhead activities, injuries and trauma from direct effects on the shoulder region, as well as wear and tear from one's age. We could also find it's caused by the wrong use of area due to positioning or posture defects.

Summary:
✅️Routine & Repetitive Activities
✅️Job activities especially overhead tasks
✅️Trauma & Injuries
✅️Wear and Tear from Age
✅️Wrong Positioning & Posture

Symptoms:

Most inflammatory symptoms come with some form of tenderness of the affected region. This may be in the form of swollen and reddish coloration of the part, which shows an infected area. More so, we may start to feel discomfort and pain from the region when we try to initiate our usual routine activities.

At this point, we will notice a drastic reduction in the range of motion when the arm region is out of use. due to stiffness and discomfort from the region. The clicking and snapping sensations are also possible symptoms each time we put our arm region to use.

Summary:
✅️Inflammation from tenderness of shoulder region
✅️Swelling and Reddish Coloration of region
✅️Reduced Range of Motion while in use
✅️Clicking and Snapping sensation while in use
✅️Pains, discomfort, and difficulty (weakness) in using the arm.

Stages of Rotator Cuff Tendonitis

It is important we note the three (3) different stages of rotator cuff tendonitis to know what commensurate treatment is applicable for an efficient treatment.

The first stage, which is the initial appearance of symptoms, comes with the presence of inflammation and slight irritation within the region. At this stage, pain and discomfort are observed to be on a very low scale.

The second stage is where we start to see the presence of a clicking and snapping sensation around the shoulder region while the arm is in use. Each time we tend to use an arm within a range of motion, this sensation is observed. This comes with relative wear and tear (partial occurrence), discomfort, and pain higher than that of the mild or first stage.

The third stage is known to be the last and most severe stage. We have a complete wear and tear of tendons around the should region. This comes with severe pain and discomfort.

How would you diagnose rotator cuff tendonitis? Any clinical investigation or assessment tests? ( Don't add treatment to this question)

Diagnosing Rotator Cuff Tendonitis

Through Patient's HistoryTaking:

While doing this, we have to ensure that we ascertain the type of activities this patient indulges in while carrying out his or her daily routine. Daily activities that expose him to repetitive use of his arm, especially overhead activities,s, are likely to make him susceptible to this condition.

It is also important to enquire about the patient's medical history to ascertain if there are other medical conditions or surgeries within this region. Past injuries and trauma in this region may cause reoccurring discomfort and pain if not properly treated in the past.

We can also ask patients where they observe this pain. To be sure if it is from the arm or the shoulder itself. The patient is likely to be exact in detecting this position.

Through Patient's Physical Examination of Region:

These have to be done by having a physical conduct of conducting of the shoulder region. This allows us to have an immediate and firsthand observation of the region without guesses. Physically examining the region would help detect swelling, irritation, and any other form of inflammation. and tenderness.

Asking the patient to carry out some mild activities like lifting or moving the arm over the head position would also help detect there is a limited range of motion within the region.

Through Carrying out Imaging Tests:

The adoption or use of the imaging test type is considered a more advanced testing option to ascertain rotator cuff tendonitis symptoms. Therefore, the use of X-ray, Ultrasound, and MRI are the best options to determine the extent of tendonitis as well as ascertain the level of damage that has occurred.

Many medical conditions use the imaging test option as one that brings precision to their result-oriented purpose for proper diagnosis and treatment. This is also seen to be beneficial while diagnosing rotator cuff tendonitis.

The arthroscopy diagnosing method that involves medical personnel could see through the joints may be useful as well.

Through Special Tests:

Rotator Cuff Tendonitis is a condition that can also be seen to be diagnosed through special tests so. as to detect the presence and stage of symptoms in the patient.

1.Neer Test:
In this testing option, the scapula is stabilized with the patient's arm stretched forward. This test tends to assess the subscapularis muscles, which are responsible for arm rotation and shoulder stabilization. When the patient feels any form of pain during this examination, then tendonitis is eminent.

2.Hawkins Kennedy Test:
The patient's elbow to shoulder is kept at a 90⁰ position before it is rotated inward. When this inward rotation is done and pain is observed, this shows impingement of the Rotator Cuff.

3.Drop Arm Test:
This is a test that entails a patient stretching its arm at a 90⁰ level and then being asked to drop it lower. When the patient finds it uneasy to drop the arm below, then we can relate this to rotator cuff tendonitis condition.

Through Physical Interventions:

The use of physical interventions is to help us make continuity from the above-listed diagnostic procedures. They help profer solutions. However, this may be likened to a treatment procedure, which shouldn't be discussed in the question. under review.

Try to practice at least 3 exercises that you have learned from the lesson. Share images, gifs, or videos while practicing, preferably gifs or videos.
Cross Body Shoulder Stretch

This is one exercise that helps stretch the muscles instantly. Having either of the arms stretched with the other complimenting in either way is quite engaging and helps improve shortened muscles. Shoulder flexibility and range of motion are seen to be improved.

Wall Slides

From the exhibition, this tends to improve the range of motion of the arm. These upward and downward stretches help in improving flexibility as well.

Shoulder Isometric Exercise

This is a resistance type of exercise that helps us build strength within the shoulder region. The back elbow is placed on a towel piece with the arm position observed at 90⁰. There is usually little or no movement during exercise.

Scapular Squeeze

This is done while sitting down with a motion to squeeze the scapula through the back and forward movement of the shoulder while keeping the arm stretched at a given angle.

Share your review after performing these exercises either on yourself, a healthy individual, or a patient.

I had this exercise performed by myself, therefore, these are my videos shared for the purpose it is meant for. Given the fact there was no underlying condition or any form of tenderness, inflammation, or swelling, I didn't observe any form of discomfort while performing the physical intervention.

Since I didn't experience any form of discomfort or pain from exercise, this was, hence, a pure exercise for me to help create flexibility and relief of the region in case there are traces of tendonitis.

I got my arm and shoulder region stretched through the given and practiced exercise. The entire muscle and tendons were energized through the practiced exercises.

Thank you, friends...

Good morning sir, I was expecting your entry as well. You've highlighted and explained more about rotator cuff tendonitis and I've learnt a lot from your diagnosis. You've performed the exercises just as the teacher did in the lesson video. I did mine yesterday though, good luck sir.

Thank you for understanding the lesson and sharing your assignment; I hope that you will enjoy this week's lesson and try to implement it in your life if you see any such case.

Observations

Task 1 (2.8/3)
You have shared a great knowledge about rotator cuff tendonitis, its symptoms, causes and it's stages. But it would be better to add the name of stages with it. I appreciate your effort.

Task 2 (3/3)
In the second question, you tell us about how you have to assess a patient by doing physical examination, history taking, special tests and investigations. . Excellent.

Task 3 (4/4)
You try the wall slides, cross body shoulder stretch, shoulder isometric and scapula squeeze. You did the wall slides, cross body shoulder stretch, shoulder isometric and scapula squeeze correctly. Always remember to apply heat pack in chronic condition and ice pack in acute condition before performing exercises to relax the muscles and reduce stiffness. I appreciate your efforts.

Overall you made a great attempt to answer all the questions. I appreciate your efforts. But next time try to avoid the above written suggestions. Keep learning and try to implement your knowledge to the people suffering from rotator cuff tendonitis or any type of shoulder issue. Thank you.
