Coarctation of the Aorta (CoA)

Varón de 54 años que ingresó por disnea de esfuerzo e hipertensión. La ecocardiografía mostró estenosis de válvula aórtica bicúspide intensamente calcificada. En la radiografía de tórax se observó inesperadamente una escotadura costal obvia y los signos típicos en el istmo aórtico (figura 1). La angiografía por tomografía computarizada reveló una coartación de la aorta (CoA) junto con hipoplasia del cayado aórtico distal a la arteria subclavia izquierda (figura 2). La arteriografía coronaria mostró una oclusión del 60% en la arteria coronaria derecha y del 70% en la arteria descendente anterior (DA)

La intervención quirúrgica se llevó a cabo mediante esternotomía media
se realizó anastomosis de la arteria mamaria interna izquierda (AMII) con la DA, y de la arteria descendente posterior (ADP) con vena safena (VS)
A continuación se provocó la parada cardiaca y se realizó la sustitución de la válvula aórtica bicúspide calcificada por una prótesis valvular mecánica

A 54-year-old male admitted for dyspnea on exertion and hypertension. Echocardiography showed intensely calcified bicuspid aortic valve stenosis. On the chest radiograph, an obvious costal notch and typical signs on the aortic isthmus were unexpectedly observed (Figure 1). Computed tomography angiography revealed coarctation of the aorta (CoA) together with hypoplastic aortic arch distal to the left subclavian artery (Figure 2). Coronary arteriography showed an occlusion of 60% in the right coronary artery and 70% in the anterior descending artery (DA)

The surgical intervention was carried out by means of median sternotomy
anastomosis of the left internal mammary artery (AMII) was performed with the DA, and of the posterior descending artery (ADP) with saphenous vein (VS)
The cardiac arrest was then caused and the calcified bicuspid aortic valve was replaced by a mechanical valvular prosthesis.
