Any over the top of the handle bars? Too many of those for me!
One I do remember, that didn't hurt as I was on grass, was after swapping bikes with a friend and finding out his brakes were on different sides to mine. What was supposed to be an 'epic skid' turned into a very amusing (for them) flight over the bars :D
We can always trust on friends to point and laugh when such things happen :)
This one happen when I was a kid. I was biking on a circular walking track and beside this track was a little swamp. On the other side of this swamp was a baseball field. Connecting the two was a small home made wooden bridge which at the time looked to be flush to the ground. I (full speed ahead) decided to zoom onto the bridge into the baseball field area. It was at this time I found out that the bridge was a few inches above ground level but I didn't notice due to the green grass that was also a couple inches long. I never did get to the baseball field as I biked back home with a massive headache after going over the top of the handle bars and smashing into the ground/wooden bridge.
