Given the rate at which we have advanced and improved arsenals, post the World Wars, it is quite understandable and agreeable that another World War would post a serious threat to human existence. The effect of the nuclear, chemical and biological weapons combines, used at a mass level should very much be the cause of human extinction.

There could be a human-only pandemic, involving one or more viruses, prions, or antibiotic-resistant bacteria, causing colossal and catastrophic loss of life

However, more deadly and most likely to cause the wipe out of the human race would be a pathogen with a wide host range in multiple species that could reach even isolated humans by using insects or other animals as "carriers".
