Medical data is one of the tools a medical personnel utilizes in offering the best healthcare to a patient, it contains vital and pertinent information regarding a patient. And this helps the medical personnel to offer the best possible treatment. About 20 years ago this data was kept in a paper form (hard copy) and this as we all know it is prone to errors and sometimes even loss of data, but recently technology has found its way into the medical system of most nation. Now technologies are being utilized, the digital age has brought about database that presents patient's information instantly and this in turn reduces error that existed during the "paper age"
Reducing error to a bare minimal is very important, and as said earlier medical data is one of the many tools utilized by the physician in delivering an in depth care to the patients. Therefore physicians and patients needs the database to be error free, so as to administer or receive a better quality care .The involved parties especially the patients want the database to be reliable, transparent and also secured . But we have to ask ourselves,

Current state is not reliable: because of the centralized nature of the current state of the medical record system, data are not reliable and it can be easily misplaced or even deleted, unauthorized personnel mostly end up viewing the data
Lack of interoperability: another state of the current system is that medical information is not transferable to another hospital, patients go to one hospital and end up repeating same test in a different hospital. This leads to loss of time, the restriction in the exchange of pertinent healthcare information or data in numerous health organizations leads to the limit at which healthcare providers and medical practitioners from these different organizations perform and give the best care to their customers.

Security, reliability and interoperability seems to be the 3 major characteristics of the current state of medical record system and this 3 characteristics needs to be solved.
