11 September 1991, as a result of non-compliance with procedures during the repair work, the entire leading edge of Continental Express Embraer 120 Brasilia was torn off from the horizontal stabilizer during flight the following day.

The aircraft lost control and then went into a dive and began to break up due to the enormous overload. The plane hit the ground at high speed about 110 kilometres from Houston, Texas. All passengers were killed.

The investigation blamed the carrier for creating the conditions for failing to follow corrective procedures.
