Religious Exemption to Controlled Substances Act, through a Doctor

If anyone has a Doctor they speak to regularly, and wants to request Religious Exemption from the Controlled Substances Act through a Doctor. Go to your doctor with a copy of Form 1571 and be very clear with them that you are requesting a Religious Exemption, not Medical, and that you want them to write that in. Show the Doctor the Documents below to explain how it works, and the link has form 1571 and instructions for Doctors.
